No I have not and I am not sure if I would or not I guess it would depend on what the surprise is.

Yes, when I was working in Canada, my supervisor and co-workers organized a surprise party for me on my birthday. I was working and suddenly, my supervisor came to my desk with a birthday cake, I was thrilled.

Yes, I have been once surprised on my birthday, my younger planned a secret birthday party and I was not aware of it until the last moment.
